By accessing or using the Living Will Medical Clinic website, booking system, or mobile application, you agree to be bound by these Terms and Conditions.
Medical Services
Our clinic provides medical consultations and advance care planning services under the supervision of Dr. Richard Sullivan, MD. Online booking does not establish a doctor-patient relationship until your appointment is confirmed and you have been seen by the provider.
Appointment Policy
Appointments may be cancelled or rescheduled up to 24 hours before the scheduled time without penalty. Late cancellations may be subject to a fee. We reserve the right to reschedule appointments due to emergencies.
